Hannah Schwartz, BSPH, is RHW’s Community Health Worker. A community health worker is a frontline public health worker who is a trusted member of and/or has a close understanding of the community served. Community Health Workers provide important services to local communities including assistance with healthcare visits, medication safety, and assisting with social needs like food insecurity and transportation. They often provide a bridge between clinical care and community resources.
